Why Do Young Men Get ED? Causes Beyond Aging

ED in men under 40 is most often driven by psychogenic factors (performance anxiety, depression, relationship stress), lifestyle factors (sleep loss, alcohol, recreational drugs, anabolic steroid use), or specific medical issues (low testosterone, thyroid disease, pelvic trauma). Vascular ED is uncommon at this age but should be screened in metabolic-risk patients.

Common drivers in younger men

Anxiety-driven ED is the leading cause under 40 and often presents as situational — fine during masturbation, difficult with a partner. Anabolic steroid use suppresses the HPG axis and is increasingly common.

Sleep apnea, untreated depression, SSRIs, and chronic cannabis or opioid use are frequently overlooked contributors.

Workup

Baseline workup includes morning total testosterone, prolactin, TSH, fasting glucose or A1c, lipid panel, and a focused mental-health screen. Penile Doppler is reserved for suspected vascular cases.

There is some evidence that high-frequency use is associated with reduced arousal in partnered sex, but causality is debated. A short reset period often helps.
